It is important to complete and return this form as early as possible - at least 6 weeks before you travel - as an appointment will be required with the practice nurse to actually receive the vaccinations. If we do not receive your form at least 6 weeks before you travel you may have to attend a local travel clinic. Depending on which vaccines you require you may need several appointments with the Practice Nurse and it is imperative therefore that we are able to contact you by telephone.  These vaccines have to be ordered on prescription. Your appointment may need to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge. This is because not all travel vaccinations are included in the services provided by the NHS.
